EQUILIFE, INC. – STORE CREDIT PROGRAM, TERMS & CONDITIONS

These Store Credit Program Terms & Conditions (“Terms”) govern the issuance and use of store credits (“Store Credit”) offered by EquiLife, Inc. (“EquiLife,” “we,” “us,” or “our”) to customers who elect to receive Store Credit in connection with a product return request.

By requesting and accepting Store Credit, you (“customer,” “you,” or “your”) agree to be bound by these Terms.

1. Eligibility

Store Credit is available to customers who request a return for eligible products in accordance with EquiLife’s Return Policy. EquiLife reserves the right to determine eligibility for Store Credit at its sole discretion.

2. Issuance of Store Credit

  • Upon approval of a return request and the customer’s election to receive Store Credit, EquiLife will issue Store Credit in an amount equal to:
    • The original purchase price of the returned product(s), along with any sales tax, excluding shipping, plus
    • An additional 10% bonus calculated on the original purchase price (excluding taxes and shipping).
  • Store Credit will be issued only after the returned item(s) are received and processed, unless otherwise determined by EquiLife in its sole discretion.

3. Non-Expiration

Store Credit issued under this program does not expire and may be used at any time, subject to these Terms. The 10% Bonus is subject to expiration after 90 days of Store Credit Issue.

4. Redemption

  • Store Credit may be redeemed exclusively on purchases made through EquiLife’s official ecommerce platform, Equi.life, and shall not apply to purchases made on any other platform or affiliated store.
  • Store Credit can be used for payment or as partial payment for the purchase of any supplement, protocol or at home lab test.
  • Store Credit cannot be applied toward:
    • Taxes
    • Shipping or handling fees
    • Prior purchases
  • If a purchase exceeds the available Store Credit balance, the remaining amount must be paid using another accepted payment method.

5. No Cash Value

  • Store Credit has no cash value and cannot be redeemed, exchanged, or converted into cash, in whole or in part.
  • Once a customer selects and receives Store Credit, this election is final and irreversible.

6. Non-Transferability

  • Store Credit is non-transferable and may only be used by the original customer to whom it was issued.
  • Store Credit may not be sold, assigned, or otherwise transferred to any other individual or entity.

7. Returns of Purchases Made with Store Credit

  • If a purchase made using Store Credit is returned, any refund will be issued in the form of Store Credit only.
  • The reissued Store Credit will reflect the amount applied to the returned purchase, in accordance with EquiLife’s Return Policy.

8. Lost, Stolen, or Unauthorized Use

EquiLife is not responsible for lost, stolen, or unauthorized use of Store Credit. Customers are responsible for safeguarding their account information and Store Credit balances.

9. Fraud and Abuse

EquiLife reserves the right to refuse, revoke, or cancel Store Credit if it suspects fraud, abuse, misuse, or violation of these Terms, including but not limited to attempts to circumvent return policies or exploit promotional benefits.

10. Modification or Termination

EquiLife reserves the right to modify, suspend, or terminate the Store Credit Program at any time, without prior notice. Any changes will not affect Store Credit already issued, except as required by law.

11. Limitation of Liability

To the fullest extent permitted by law, EquiLife shall not be liable for any indirect, incidental, consequential, or punitive damages arising out of or related to the Store Credit Program.

12. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict of law principles. You agree and consent to jurisdiction of the courts in Palm Beach County, Florida for any disputes.

13. Alternative Dispute Resolution and Binding Arbitration

In the event of any dispute, claim, or controversy arising out of or relating to these Terms, the parties shall first attempt in good faith to resolve such dispute through informal negotiations.

If unresolved within thirty (30) days, the parties agree to participate in non-binding mediation. If still unresolved within sixty (60) days, either party may elect binding arbitration administered by the American Arbitration Association (AAA).

The arbitration shall take place in Palm Beach County, Florida before a single arbitrator unless otherwise agreed. Each party shall bear its own costs, and the arbitrator shall not award punitive damages unless permitted by law.

The arbitrator’s decision shall be final and binding. The parties waive the right to a jury trial and agree disputes will be handled individually, not as a class action.

Either party may seek temporary injunctive relief in Palm Beach County courts if necessary.

14. Severability

If any provision of these Terms is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
